The Doctors and Prevention magazine are searching for someone who inspires others to take control of their health.

Learn how you can win $5,000 and a trip to Hollywood, California to appear on The Doctors at Prevention magazine.

Are you or someone you know the picture of health? Did you overcome a deadly diagnosis? Have you transformed your body or helped others improve their health?

The winner will receive $5,000, plus $5,000 for the charity of his or her choice, appear in Prevention magazine, and win a trip to Hollywood, California, to share their story on The Doctors.

Void where prohibited. Must be over 18 years of age and legal resident of 49 United States or DC (excluding residents of AZ) to enter or to nominate others.

Essay and testimonial video will be judged on most compelling, empowering health story and appearance of good health.
