Adult bed handles recalled after three deaths


Adult portable bed handles have been recalled by Bed Handles Inc., due to entrapment and strangulation hazards; three deaths have been reported. 

The U.S. Consumer Product Safety Commission (CPSC) and Bed Handles Inc., of Blue Springs, Mo., are announcing the voluntary recall of about 113,000 adult portable bed handles.  When attached to an adult’s bed without the use of safety retention straps, the handle can shift out of place creating a dangerous gap between the bed handle and the side of the mattress. This poses a serious risk of entrapment, strangulation and death.

Safety advisory posted after four deaths from HeartMate II® LVAS Pocket System Controller

Medical device not being recalled

After four deaths and five reports of loss of consciousness or other symptoms of hypo-perfusion, Thoratec Corporation initiated a voluntary worldwide medical device correction to update its labeling and training materials for the HeartMate II® LVAS Pocket System Controller (the “Pocket Controller”). This safety advisory is being issued because some patients and caregivers have experienced difficulties with the process of changing from a primary system controller to their backup system controller.

Five infant deaths involving the Nap Nanny and Chill Infant Recliners by Baby Matters LLC lead
to recall; CPSC, firm settle administrative litigation

There has been 92 incident reports, and five infant deaths

The U.S. Consumer Product Safety Commission (CPSC), and Baby Matters LLC of Berwyn, Pa., is announcing a voluntary recall of all models of its Nap Nanny and Nap Nanny Chill infant recliners and covers. This recall is announced as part of the settlement of an administrative case filed by CPSC in December 2012, which sought a mandatory recall of the Nap Nanny and Chill products.
